Pre-K screenings will be held at Martin Luther King Elementary School on:

GENERAL POLICY 
According to the School Code of Illinois, no child may be enrolled in the public schools without proof of a recent medical examination and immunization record. THE BOARD OF EDUCATION DOES NOT ALLOW STUDENTS TO BE ENROLLED WITHOUT PROPER MEDICAL AND IMMUNIZATION RECORDS. A child must be five years old on or before September 1 of the school year in which he or she enrolls. The District requires that all parents or guardians show proof of residence before registering students.
 
SPECIFIC GUIDELINES
I. All students enrolling or registering for the 2017-2018 school year must adhere to the following guidelines:
a. The student must be enrolled or registered by a parent/foster parent/surrogate parent or guardian and must be a resident of District 147.
b. Parents must show picture identification with legal name. In the case of a change in name as a result of married or divorce status, the custodial parent must show appropriate legal documentation.
c. Legal guardians, surrogate parents, and foster parents (for whom this district has been established as the resident district of the student) must show Illinois Public Agency or Illinois court documentation as well as identification.
d. Caseworkers must show picture identification from the agency they represent. Foster parents must also show proof of residence and should accompany the caseworker to the school.
II. All parents and guardians must show two (2) forms of identification as proof of residence limited to the following documentation:
(1) Driver’s license with current address; or
(2) State ID card with current address
AND
(3) Current utility bill or printout indicating that the utility has been applied for (with current address being the same as the address on the photo I.D.)
(4) Medical/insurance card with current address
The following will not be accepted as proof of residence:
(1) Leases
(2) Rent receipts
(3) Cable bill
(4) Voter’s registration card
(5) Temporary documents from the Department of Public Aid
 
PLEASE NOTE: No other documentation will be accepted without the approval of the Interim Superintendents or the Director of Curriculum, Instruction and Assessment.
 
THE RESIDENT DISTRICT OF A HOMELESS STUDENT IS THE ILLINOIS DISTRICT IN WHICH THE STUDENT ENROLLS FOR EDUCATIONAL SERVICES. HOMELESS STUDENTS INCLUDE INDIVIDUALS AS DEFINED IN THE STEWARD B. MCKINNEY HOMELESS ASSISTANCE ACT.
 
III. School personnel must also require that the following documents be presented at the time of enrollment/registration:
a. Required health records including physical exams (Early Childhood, Pre-K, and Kindergarten for initial enrollment only and 5th graders) and mandated immunizations
b. Birth certificate which should be the original document or a certified copy
c. IEP, transfer or other official school documentation if the child was previously receiving special education services
IV. Parents will be assessed for payment of tuition for non-resident students in an amount not exceeding 110% of the per capita tuition cost of maintaining the schools of the district for the preceding school year. Students who become nonresident during a school term shall not be charged tuition for the remainder of the school term in which they became nonresident pupils pursuant to the School Code of Illinois.
V. Textbooks for students unable to buy them, including but not limited to children eligible for free lunches or breakfasts under the Community School Lunch Program, shall be loaned only, and shall be returned at the end of each school term pursuant to the School Code of Illinois.
Vl. See school policy for fees. Waiver of school fees for students unable to afford them shall be pursuant to school board policy.
VlI. All students, with the exception of Early Childhood Special Education and Pre-kindergarten students, are required to wear school uniforms pursuant to school board policy.

TRANSFER OF STUDENTS OUTSIDE OF DISTRICT
Students moving out of West Harvey-Dixmoor Public School should contact the building secretary to request a Transfer out of District.
 
ANNUAL FREE SCREENING

West Harvey-Dixmoor Public School, District #147 will be holding the ANNUAL FREE SCREENING of children between the ages of 3 through 5. Please see above for dates and times.

Please bring with you at the time of screening the child’s original birth certificate; proof of residency (utility bills, driver’s license, etc.), necessary immunization documents, and physical and dental examination records. Proof of lead screening and Hepatitis-Medical card or social security number.

Your child will be screened in the areas of gross and fine motor skills, language, visual discrimination and memory.
 
Placement eligibility is as follows:
Kindergarten — 5 years of age on or before September 1, 2017
Pre-K — age 4 on or before September 1, 2017
Waiting List — age 3 on or before September 1, 2017
