Case Management Services
Smart Talk Case Management Services, is a prevention program that provides support for the development of children from birth to three years of age by focusing on the child and family through a network of child and family service providers.
Smart Talk Case Management provides developmental screening for children up to 36 months of age and develops an individualized family service plan for them and their families. Once service needs are determined and a plan is developed, Smart Talk's case manager will proveide follow along and work with the family to help them meet their goals.
